Forms

The Forms tab can be used to create simple forms that customers fill out with their purchases for data collection or more complex registration. Forms can be required or optional and can be set per purchase or per ticket.


To create a new form:

  1. Click the Forms section in your left side navigation panel
  2. Click Create in the upper right corner
  3. A new row will appear in your Forms list. Click the new row to edit it.




To edit a form:

  1. Update the Title and Description/Instructions for the new form. This information will not be visible to the customer.
  2. Check the checkbox if you would like to show the question and responses on the customer's order.
  3. Click Save.
  4. Enter your question, select the answer type from the dropdown menu, and click Save.

    - Short Answer

    - Multiple Choice

    - Info Image

    - Image Upload

  5. Check the box if you would like the question or form to be Required on all purchases.

  6. Click Add Question to add more questions.

  7. To delete a question, click the trash can icon.

               


Set Rules for Your Questions

  1. Click Rules.
  2. Select Once per purchase or Once per ticket.

    - Once per purchase: Your customer will be asked to fill out this form one time, regardless of the number or types of tickets purchased.

    - Once per ticket: Your customer will be asked to fill out this form multiple times, once for each ticket purchased.

  3. Create Matching Rules to add a question to a specific event, ticket section, or price level by selecting the matching details from the dropdowns.

  4. Click Save.





View Your Form Responses

  1. Click Responses
  2. Filter responses by event by selecting a specific event from the dropdown menu, or view responses from All Events
  3. Select Individual to view one response at a time or Summary to view an overall summary of responses.
  4.  Click Download or Print 





Your form will be shown on the checkout page before the customer completes their purchase. An example can be seen below:
